A NOTE FROM THE NURSE:
Attention Parents of Grade 9 Students!
Each year the Public Health Program offers the TDAP Vaccines to all grade 9 students.
Immunizations in Manitoba are voluntary and only given with the consent of the parent or legal guardian. Your child will be sent home with important information for your review. Please read the immunization fact sheet, complete the consent form and return the form to school with your child as early as possible. Consent forms should be sent back to the school by February 8, 2016.
Please return Consent Form to the school even if you do not want your child immunized. Complete the form with your child’s name, date of birth, PHIN, and MHSC. Parents are asked to answer all the health questions, sign and date the form, provide a contact phone number and check off yes or no consent.
At Gordon Bell High School, students will be immunized on April 6, 2016 in the afternoon.
If your child is not immunized at the school clinic, a catch-up clinic will be offered between April 19th to 22, 2016 at the school.
